Our client is looking for a full-time Admissions Coordinator to join their growing Patient Services team. They operate in the healthcare industry, delivering coordinated care and telehealth services to patients across the U.S. Over the past few years, they’ve expanded rapidly, serving thousands of patients while maintaining a patient-first philosophy and streamlining cross-functional communication between internal departments. Responsibilities: Referral Intake & Review - Review and triage patient referrals from various healthcare sources (e.g., Careport, SNFs, CHHAs, LHCSAs). - Determine appropriate intake scripts based on type of services, location, and insurance. - Confirm service eligibility and review documents like TCM, CCM, and DME referrals. - Present patients with additional applicable services and effectively encourage them to consider and opt into those offerings. - Documentation Management - Collect and verify all necessary intake documents (e.g., facesheets, medication lists, discharge summaries). - Ensure documents are properly entered into the CRM system. Patient Communication & Scheduling - Use friendly and professional communication to schedule services and follow scripted calls. - Coordinate with provider availability using the Emma Care Calendar. - Schedule appointments efficiently based on service type and urgency. Call Outcomes & Follow-Up - Log call results in the CRM, noting no contacts, refusals, transfers, expirations, etc. - Escalate complex or unresolved cases to Account Managers in real-time. - Conduct cold calling to prospects, reaching out to individuals who may not be expecting the call. - Engage in sales-related activities by presenting and offering the client’s product when appropriate, making sales an important component of the role even though it is not a full sales position. Compliance & Consent - Handle patient consent processes via email or Dropbox. - Maintain compliance with documentation regulations specific to each state. Interdepartmental Coordination - Communicate scheduled appointments to Account Managers. - Work closely with Clinical, DME, Therapy, and Compliance teams to ensure patient readiness. Requirements - High School Diploma or GED required. - At least 2 years of experience in healthcare admissions, patient intake, or care coordination. - Clear spoken English with a very mild or neutral accent. - Ability to work from 8:00 AM to 5:00 PM Qualifications - Associate’s or Bachelor’s degree. A degree in healthcare-related fields is a plus. - While this is not a sales role, strong communication and sales-related skills are highly valued, as well as willingness to offer additional services when applicable. - Strong communication and organizational skills. - Ability to multitask in a fast-paced environment. - Comfortable navigating EHR systems, calendars, and CRM tools. - Knowledge of U.S. health insurance (Medicare, Advantage Plans). - Understanding of NY-specific healthcare documentation (e.g., NY M11Q, CFEEC) is a plus.
